Could there be a negative tarot card in the greeting cards?

The idea of tarot having negative cards is a common misconception. All cards carry a wide variety of meanings dependent on the reader's lens at the moment. That said, because these greeting cards may be gifted to those with varying knowledge of tarot, the cards included have been intentionally curated.

So don't worry, no one will be getting the Death card for their Birthday. The cards used are ones whose themes translate warmly and clearly in a standalone message to even the most novice of readers — reflection, encouragement, growth, support, etc.

Is it the same card inside every time?

Not at all! Much like my tarot reading to gos, the cards are shuffled and inserted at random.
